Our intake team will email your intake packet which should be completed before your first session. Forms include:

1. Treatment Consent and Authorization Form

2. Release of Information

3. Symptom Checklist

4. HIPPA Form

Insurance and ID

1. In-person sessions, bring copies to your first session

2. Telehealth sessions, take pictures and email them to billing.embranch@gmail.com

1. You can request a therapist or our intake team can select for you.

2. Your assigned therapist will contact you for scheduling.
